The 100Plus Remote Monitoring Program is a remote patient monitoring platform empowering doctors and nurse practitioners to manage your chronic cardiac conditions remotely and proactively monitor your blood pressures and weights with the hopes of avoiding expensive, episodic care and driving a higher quality of life. When a Medicare patient receives a 100Plus medical device, it comes with a small co-pay, comes fully configured, and is ready to use out of the box –

Billing

Financial and Managed Care Policies: 

  • Your insurance policy is a contract between you and your insurance company. It is your responsibility to know your plan. 
  • All copay, co-insurance, and other balances due are required at the time of each visit. If you have a deductible on your insurance plan, payment will be required at each visit until it is met. 
  • We accept all major credit cards and checks. A fee will be charged for all returned checks. 
  • We are contracted with many insurance companies and other health plans to accept assignment of benefits. We will bill those insurance companies; however, you will still be responsible for your co-payments, co-insurance, and deductibles. Not all insurance plans cover all services. If your insurance plan considers a service not covered, you will be notified prior to your service to make arrangements for the charge. 
  • We will bill your insurance company for all services provided in the hospital. You are responsible for any balance due. 


Cancellation and “No Show” Fee Policy:

  • Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, HeartCare Consultants reserves the right to charge a fee of $50 for established patients and $125 for new patients.
  • “No Show” fees are billed to the patient. This fee is not covered by insurance and must be paid prior to your next appointment. Multiple no-shows in any 12-month period may result in termination from our practice. 
  • Kindly give us 24 hours’ notice if you are not able to make your appointment. 
  • If you arrive more than 10 minutes late for your appointment, it may need to be rescheduled depending on schedule availability.
